Long COVID is a recognised post-infectious condition that can affect multiple systems in the body. Symptoms and their underlying mechanisms vary between individuals, and research is continuing into factors including immune dysregulation, inflammation, vascular changes and neurological effects. Persistent or worsening symptoms should be appropriately medically assessed.

The Science

How HBOT May Support Long COVID Recovery

Long COVID can affect multiple systems in the body and may involve persistent fatigue, brain fog, breathlessness, pain, sleep disruption and reduced physical capacity. The underlying biology is complex, with research exploring factors including inflammation, vascular changes, neurological effects and altered cellular function.

Hyperbaric Oxygen Therapy significantly increases the amount of oxygen dissolved in the blood plasma. Under pressure, this temporarily increases oxygen availability throughout the body, including to tissues with high metabolic demands such as the brain and muscles. This may influence several of the biological processes relevant to recovery from persistent post-viral symptoms.

Long COVID is complex and varies considerably between individuals. HBOT isn't a cure and should be considered as a potential adjunct alongside appropriate assessment and management of individual symptoms.

May Support Cellular Energy

Oxygen is essential for mitochondrial production of ATP. Increasing oxygen availability may support cellular energy processes relevant to physical and mental fatigue.

May Support Cognitive Function

The brain has high oxygen and energy requirements. HBOT may support neurological metabolism and neuroplasticity — processes relevant to cognition, concentration and recovery from neurological symptoms.

May Influence Inflammation

Long COVID may involve persistent inflammatory and immune-system changes in some people. HBOT can influence inflammatory and oxidative pathways that may be relevant to these processes.

Supports Tissue Oxygenation & Recovery

HBOT substantially increases dissolved oxygen in the blood plasma, increasing oxygen availability to tissues during treatment and potentially supporting normal cellular repair and recovery processes.

The Research

What Does the Research Say?

HBOT has been studied as a potential treatment for Long COVID, including in randomised sham-controlled trials. Some studies have reported improvements in cognitive function, fatigue, sleep, pain and quality of life, while systematic reviews continue to highlight the relatively small evidence base and the need for larger, standardised trials. The results are encouraging in some areas, but HBOT should not yet be presented as a proven treatment for every person or every symptom associated with Long COVID.

2022 Randomised, Double-Blind, Sham-Controlled Trial · 73 Patients

Researchers randomised 73 people with ongoing post-COVID symptoms to either 40 HBOT sessions or sham treatment. Compared with the sham group, the HBOT group showed statistically significant improvements in global cognitive function, attention and executive function. Improvements were also reported in energy, sleep, psychological symptoms and pain interference.

Important limitation: This was a relatively small trial involving 73 participants, and follow-up immediately after treatment does not establish how broadly or consistently the results apply to people with different Long COVID symptoms.

2025 Systematic Review · 7 Studies · 199 Participants

Researchers reviewed seven studies from seven countries involving 199 people with Long COVID. The studies included both randomised and observational research and used between 10 and 60 HBOT sessions. Across the included studies, improvements were reported in areas including memory, executive function, attention, fatigue and pain.

Important limitation: Treatment protocols varied considerably and the total number of participants remained small. The authors concluded that larger randomised studies are needed to establish efficacy more confidently.

2024 Systematic Review · 10 Clinical Studies

This review identified 10 clinical studies examining HBOT for Long COVID, including three completed randomised controlled trials. Most included studies reported improvements in some areas such as quality of life, fatigue, cognition, neuropsychiatric symptoms or cardiopulmonary function.

Important limitation: The studies varied considerably in design and quality, and the researchers concluded that rigorous, larger-scale randomised trials are still needed to establish appropriate indications, protocols and longer-term outcomes.

What does this mean for Long COVID? Current research provides some encouraging evidence for HBOT in areas including cognition, fatigue, sleep and quality of life. However, Long COVID is complex and the evidence base remains relatively small. HBOT is therefore best considered as a potential adjunct rather than a guaranteed solution.

Is there scientific evidence for HBOT in Long COVID?

Yes, HBOT has been studied specifically in people with Long COVID, including in randomised sham-controlled trials. Some studies have reported improvements in areas including cognition, fatigue, energy, sleep, pain and quality of life. However, the evidence base remains relatively small and further large-scale research is needed before HBOT can be considered an established treatment for Long COVID.

Can HBOT help with Long COVID fatigue and brain fog?

Fatigue and cognitive difficulties have been measured in Long COVID HBOT research. A randomised sham-controlled trial reported improvements in energy and several areas of cognitive function following HBOT, and systematic reviews have identified similar positive findings across some studies. However, individual responses vary and improvement cannot be guaranteed.

Are the effects of HBOT for Long COVID long-lasting?

Long-term evidence remains limited. One follow-up study involving 31 participants from an earlier HBOT trial reported that improvements across several quality-of-life and sleep measures were still present approximately one year after treatment. This is encouraging, but larger independent studies are needed to understand how consistently benefits persist.

How many HBOT sessions are needed for Long COVID?

There is no universally established HBOT programme for Long COVID. Published studies have used different treatment protocols and numbers of sessions, so research protocols shouldn't automatically be treated as a prescription for every individual. Is HBOT safe for people with Long COVID?

HBOT is generally well tolerated, but it isn't suitable for everyone. Possible side effects include pressure-related ear or sinus discomfort, temporary vision changes and other less common complications. Long COVID can also involve different organ systems and medical issues, so health history and suitability should be considered before starting HBOT.

Can I use HBOT at home?
