Researchers are studying whether this temporary increase in oxygen availability can influence biological processes involved in muscle recovery, tissue repair, inflammation and brain recovery. For rugby players, the research is relevant because the sport combines several different recovery demands: exercise-induced muscle damage, soft-tissue and ligament injury, cumulative fatigue and the possibility of concussion. What does the evidence currently suggest?Some studies and recent reviews suggest HBOT may influence certain markers of muscular and post-exercise recovery. There is also a small rugby-specific study suggesting a possible role following knee ligament injury. Research into persistent post-concussion symptoms has produced promising findings in some trials, but results across the wider evidence base remain mixed. The Science Why Oxygen Is Relevant to Recovery Oxygen is fundamental to cellular energy production and many of the processes involved in repairing damaged tissue. During HBOT, increased pressure allows substantially more oxygen to dissolve directly into the blood plasma than under normal atmospheric conditions. That does not mean HBOT automatically repairs an injury. Oxygen still needs to be delivered through functioning circulation, and recovery depends on the injury, training load, rehabilitation, sleep, nutrition and many other factors. Greater Oxygen Availability HBOT temporarily increases the amount of oxygen carried in plasma, increasing oxygen availability to perfused tissues during treatment. Cellular Signalling Hyperbaric oxygen can influence oxygen-sensitive signalling pathways involved in inflammation, oxidative signalling and cellular responses. Tissue Repair Oxygen plays an important role in fibroblast activity, collagen production and other processes involved in normal tissue repair. Blood-Vessel Formation Repeated hyperbaric oxygen exposure has been studied for its effects on angiogenesis — the formation of new microvasculature in healing tissue. Rugby Recovery What Does the Research Say About Muscle Recovery? This is one of the most relevant areas for rugby. Repeated sprinting, acceleration, tackling and contact can contribute to muscular fatigue and exercise-induced muscle damage. The evidence is not completely consistent, but several recent reviews give us a clearer picture than the older individual studies often quoted in HBOT marketing. 2025 Systematic Review & Meta-Analysis · 299 Participants Exercise-Induced Muscle Injury & Soreness A systematic review and meta-analysis examined 10 studies involving 299 participants. The authors reported evidence suggesting HBOT may accelerate recovery from exercise-induced muscle injury. However, outcomes varied according to the measure being assessed, and the studies used different pressures, treatment schedules and exercise protocols. What this means: potentially encouraging for some measures of muscle recovery, but it does not establish a universal post-match HBOT protocol for athletes. View the research on PubMed → 2026 Systematic Review & Meta-Analysis HBOT as an Athletic Recovery Intervention A more recent systematic review and meta-analysis examined HBOT specifically as an intervention for athletic recovery. The research suggests HBOT may affect some post-exercise recovery measures, but the evidence remains dependent on protocol, timing and the particular outcome being measured. The important point is that recovery markers and performance outcomes are not the same thing. An improvement in one biological measure does not automatically mean an athlete will run faster, jump higher or perform better in the next match. View the research on PubMed → 2026 Systematic Review · 19 Studies · 612 Patients The Wider Musculoskeletal Evidence Is Still Mixed A 2026 systematic review examining HBOT across musculoskeletal conditions found the strongest consistency in certain bone conditions. Evidence relating to exercise-induced muscle damage was limited, ligament evidence remained preliminary and tendon findings were inconclusive. This is why HBOT should be considered a potential adjunct to rehabilitation rather than a replacement for physiotherapy, strength work, load management or normal sports-medicine care. View the research on PubMed → Rugby-Specific Research There Is One Particularly Relevant Rugby Study One study looked specifically at professional and semi-professional Japanese rugby players with grade 2 medial collateral ligament injuries of the knee. Thirty-two players were compared. Sixteen received HBOT during the acute phase and sixteen did not. The researchers reported lower pain scores immediately after HBOT sessions and a shorter average return-to-play period in the HBOT group: approximately 31 days compared with approximately 42 days in the non-HBOT group. The important limitation This was a relatively small, non-randomised comparative study. It is interesting and directly relevant to rugby, but it does not prove that HBOT will shorten recovery time for every ligament injury or every rugby player. Concussion is a brain injury, and suspected concussion in rugby requires appropriate medical assessment and established return-to-play protocols. Researchers have studied HBOT mainly in people with persistent post-concussion symptoms after mild traumatic brain injury — rather than as an immediate treatment that allows an athlete to return to play more quickly. Randomised Controlled Research Some Studies Have Reported Improvements A randomised controlled crossover study examining persistent post-concussion syndrome reported improvements in symptoms and cognitive measures following a course of HBOT. View the study on PubMed → 2023 Double-Blinded Pilot Study But the Evidence Is Not Uniform Other controlled studies have produced less definitive findings, which is one reason HBOT for persistent post-concussion symptoms remains an area of ongoing research rather than an established universal concussion treatment. View the research on PubMed → For rugby players, this distinction matters. HBOT should never be used to bypass concussion protocols or medical clearance. A player with suspected concussion should be removed from play and managed according to appropriate medical and return-to-play guidance. HBOT may be explored as a supportive option in selected circumstances — particularly where symptoms persist — but not as a substitute for concussion care. The Research Doesn't Support That Simple Claim This is where the original article went too far. Recovery and performance are related, but they are not interchangeable. A double-blind randomised controlled trial in elite youth football players examined a single HBOT session following a 90-minute match. Researchers found no significant difference between HBOT and control groups in the measured biochemical or physical-performance outcomes. The HBOT group did show a difference in one subjective recovery measure, but the study did not demonstrate a clear improvement in speed, jumping performance or muscle-damage markers. So while HBOT may be explored as part of an athlete's wider recovery strategy, it would be misleading to describe it as a proven performance enhancer. HBOT is best thought of as one possible component of a wider recovery programme — not a replacement for the fundamentals that determine recovery and performance. Worth Exploring HBOT may be considered where athletes are managing demanding training loads, certain injuries or longer-term recovery needs, particularly when regular access can be integrated sensibly into the wider programme. Not a Shortcut It does not replace rehabilitation, sleep, nutrition, load management, strength and conditioning, medical treatment or appropriate time away from sport. Protocol Matters Research uses very different chamber pressures, oxygen exposures, session durations and treatment schedules. That makes generic “one protocol suits everyone” claims difficult to justify. Most people tolerate appropriately delivered HBOT well. Good screening and operating procedures are designed to identify potential problems before a session begins and reduce avoidable risk during treatment. The most familiar issue for many first-time users is simply the change in pressure as the chamber begins to pressurise. What Makes HBOT Safe? Safety Has Three Parts The Person Medical history, contraindications, medications and ability to tolerate pressure should be considered first. The Protocol Pressure, oxygen exposure, session duration and frequency should be appropriate for the individual and chamber. The Environment Equipment, oxygen safety, ignition controls, operating procedures and support all matter. Your First Session What Does HBOT Usually Feel Like? During Pressurisation You will usually notice pressure changing in your ears as the chamber pressurises. It is often compared with the sensation during aircraft take-off or landing. You are shown how to equalise your ears before starting. Once at Pressure Once the chamber reaches its operating pressure, the pressure-change sensation settles. The remainder of the session is generally calm, with the chamber maintaining a stable environment until decompression begins. Pressure & Equalisation Why Your Ears Matter The middle ear contains air, so pressure needs to equalise as the surrounding chamber pressure changes. Middle-ear barotrauma is one of the best-known pressure-related complications of HBOT, particularly if someone cannot equalise effectively. This is why you should never feel that you have to “push through” painful ear pressure. Pressurisation can be slowed or stopped and the issue addressed. Most recognised HBOT side effects are related either to changing pressure or increased oxygen exposure. More Commonly Discussed Ear or Sinus Discomfort Pressure changes can cause ear or sinus discomfort and, if equalisation is unsuccessful, barotrauma. Screening and learning how to equalise are important preventative steps. Individual Response Confinement Anxiety Some people feel anxious in an enclosed chamber. Familiarisation, understanding how the chamber works and knowing how to communicate or end a session can make a significant difference. Repeated Exposure Temporary Vision Changes Temporary myopic changes can occur during repeated hyperbaric exposure. These are recognised oxygen-related effects and are generally reversible after HBOT stops. Uncommon but Important Oxygen Toxicity High oxygen partial pressures can produce neurological or pulmonary oxygen toxicity. Modern protocols manage oxygen exposure to reduce this risk, but it remains one reason pressure and duration should be properly controlled. Screening Before HBOT Who Needs Additional Assessment? Screening does not mean looking for reasons to exclude everybody. It means identifying circumstances where HBOT needs to be avoided, delayed or adapted. Lung Conditions Certain lung conditions can increase the risk of pressure-related complications and require individual assessment. Cold, Sinus or Ear Problems Congestion can make pressure equalisation more difficult and increase barotrauma risk. Medications Some medications require specific consideration before hyperbaric exposure. Implanted Devices Not every implanted or medical device is rated for use under hyperbaric pressure. Seizure Risk Factors that lower seizure threshold should be considered alongside oxygen exposure. Confinement Anxiety Claustrophobia or anxiety should be discussed before entering the chamber. Untreated pneumothorax is the recognised absolute contraindication to HBOT. Other conditions are assessed individually rather than being treated as automatic exclusions. In oxygen-enriched environments, materials can ignite more easily and burn more rapidly. Thirty-five healthy, independently living adults aged 64 and over completed a protocol involving 60 daily HBOT exposures. Researchers reported increases of more than 20% in telomere length in several immune-cell populations following the intervention, alongside reductions in some senescent-cell populations. These findings attracted considerable attention because telomere shortening and cellular senescence are recognised features of biological ageing. Importantly for competitive sport, beneficial effects were reported in subgroup analyses involving elite athletes as well as college students, and across protocols using pressures both above and at or below 2 ATA. The analysis did not find a significant overall improvement in exercise-induced muscle soreness — showing that recovery markers and the feeling of soreness are not necessarily the same thing. A different 2025 study investigated HBOT in women classified as poor ovarian responders — another specific challenge encountered during assisted reproduction. The pre-post cohort included 41 women aged 20–45 and compared ovarian stimulation outcomes before and after HBOT. Researchers reported increases in the number of oocytes retrieved, normally fertilised oocytes and available embryos after HBOT. They also reported improvements in measures including follicle output rate and the available embryo rate. Interestingly, their analysis suggested oocyte numbers increased after two HBOT sessions, plateaued around four to seven sessions and did not show further increases beyond eight sessions. This gives researchers a useful signal to investigate further — but because this was a pre-post cohort rather than a large randomised controlled trial, confirmation in stronger controlled studies is still needed. Most of the included studies reported improvements in one or more areas including quality of life, fatigue, cognition, neuropsychiatric symptoms and cardiopulmonary function. The authors concluded that HBOT had shown benefits across Long COVID symptoms while calling for larger rigorous trials to refine indications, protocols and follow-up. HBOT protocols varied considerably, with pressures ranging from 2.0 to 2.5 ATA and programmes ranging from 10 to 60 sessions. Four RCTs involving 163 participants were included. The pooled analysis found significant improvements in both the Fibromyalgia Impact Questionnaire (FIQ) and tender-point count. The pooled pain result itself did not reach statistical significance, illustrating that different fibromyalgia outcomes have not always moved in exactly the same way across trials. The Latest Evidence What Does the 2026 Review Add? A 2026 systematic review widened the lens beyond fibromyalgia and looked at randomised controlled trials of HBOT across chronic pain syndromes. It included 11 RCTs involving 480 participants. Eight of those trials investigated fibromyalgia, with additional research involving complex regional pain syndrome, trigeminal neuralgia and radiation-induced brachial neuropathy. In fibromyalgia, the reviewers found consistent within-group improvements in pain, functioning and quality of life, with some studies also showing benefits when compared directly with control groups. The authors concluded that HBOT shows potential for selected chronic pain syndromes, particularly fibromyalgia. At the same time, the review identified differences in study quality, protocols and long-term follow-up. Those are important questions for future research because we still need better answers around which protocols work best and how long improvements may persist. The researchers reviewed evidence across areas including diabetic and vascular wounds, reconstructive and plastic surgery, burns, trauma, orthopaedic injuries and radiation-related tissue damage. The review describes how increased dissolved plasma oxygen creates a temporary high-oxygen environment that can support fibroblast activity, collagen cross-linking, angiogenesis and host antimicrobial defence.
