The Science

How HBOT May Support Brain Recovery

The brain has a high demand for oxygen and energy. Following a concussion or traumatic brain injury, a complex series of changes can occur involving brain metabolism, blood flow, inflammation and cellular function. These processes can differ depending on the type and severity of injury and how much time has passed.

Hyperbaric Oxygen Therapy significantly increases the amount of oxygen dissolved in the blood plasma. Under pressure, this creates a temporary increase in the oxygen available to tissues throughout the body, including the brain. In the context of brain injury, this may support or influence some of the biological processes involved in recovery.

HBOT is not a guaranteed treatment for persistent concussion symptoms, and individual responses vary. It should be considered alongside — not instead of — appropriate medical care and rehabilitation.

Increases Oxygen Availability

HBOT significantly increases dissolved oxygen in the blood plasma, increasing the oxygen available to brain tissue during treatment.

May Influence Inflammatory Processes

HBOT may influence inflammatory and oxidative processes that can form part of the body's response following brain injury.

May Support Neuroplasticity

Increased oxygen availability under pressure may influence processes involved in neuroplasticity — the brain's ability to adapt, reorganise and form new connections.

May Support Brain Metabolism & Recovery

By temporarily increasing oxygen availability, HBOT may support cellular energy and metabolic processes involved in brain function and recovery.

The Research

What Does the Research Say?

HBOT has been studied in people experiencing persistent symptoms following concussion and traumatic brain injury, including in randomised, double-blind and sham-controlled trials. Some studies have reported improvements in symptoms and cognitive or functional outcomes, while other well-controlled trials have found no significant benefit compared with sham treatment. Differences in injury type, treatment protocols, study populations and control methods continue to make this an active and sometimes debated area of research.

2025 Double-Blind Randomised Trial · 47 Participants

Adults experiencing persistent symptoms following non-stroke brain injury received 40 HBOT sessions or 40 sham sessions over 12 weeks. At 13 weeks, the HBOT group showed a significantly greater improvement in overall neurobehavioural symptoms than the sham group. Improvements were also reported in measures including sleep difficulties, vestibular complaints, anxiety and smell.

Important limitation: The study was relatively small, with 47 participants included in the analysis, and included different forms of non-stroke brain injury rather than concussion alone.

2025 Systematic Review & Meta-Analysis · 250 TBI Patients

Researchers analysed four studies involving 250 people with neurocognitive impairment following traumatic brain injury. The pooled findings reported improvements across several measured areas, including memory, attention, general cognition, executive function, information-processing speed and motor skills.

Important limitation: The evidence base was small, the included studies varied in design, and the authors concluded that larger trials using standardised HBOT protocols are still needed.

Randomised Sham-Controlled Trial · 60 Participants

A double-blind trial involving 60 military service members with persistent symptoms following mild traumatic brain injury compared different HBOT oxygen exposures with sham treatment. The primary analysis found no significant benefit from HBOT compared with sham treatment for persistent post-concussion symptoms.

Why this matters: Other sham-controlled military studies have also produced limited or mixed findings, demonstrating that the evidence for HBOT after concussion is not uniformly positive.

What does this mean for concussion recovery? There are encouraging findings suggesting HBOT may support some people with persistent symptoms following brain injury, but the research is mixed and does not show that everyone will benefit. Can HBOT help with concussion symptoms months or years after the injury?

HBOT has been studied in people with persistent symptoms months or even years after traumatic brain injury. Some studies have reported improvements in symptoms or cognitive function, while other sham-controlled trials have not found a significant HBOT-specific benefit. How long ago the injury occurred is therefore only one factor to consider, and improvement cannot be guaranteed.

What concussion symptoms have been studied in HBOT research?

Research has examined a range of persistent post-concussion symptoms and outcomes, including headaches, dizziness or vestibular complaints, sleep, cognition, memory, attention, mood and overall neurobehavioural symptoms. Results vary between studies and individuals.

How many HBOT sessions are needed after concussion or brain injury?

There is no universally established HBOT programme for persistent concussion symptoms. Research protocols have often used structured courses of multiple sessions, but these research protocols shouldn't automatically be treated as a prescription for every person. ReWell guides frequency, pressure and pacing around individual circumstances and intended use.

Can HBOT be used alongside concussion rehabilitation or physiotherapy?

HBOT may be explored as an adjunct to a wider recovery programme rather than a replacement for medical care or rehabilitation. Depending on your symptoms, that wider programme may involve your GP or specialist as well as physiotherapy, vestibular rehabilitation, occupational therapy or other appropriate care.

Is HBOT safe after concussion or brain injury?

HBOT is generally well tolerated but isn't suitable for everyone. In two randomised trials involving 143 participants and 4,245 chamber sessions for persistent post-concussion symptoms, chamber-related adverse events were uncommon, with minor barotrauma the most frequently reported and no serious chamber-related adverse events reported. Your individual health history and suitability still need to be considered before starting.
