At its best, it is simply a more deliberate way of understanding your body, making changes and learning from the results. At its worst, it can become expensive experimentation dressed up as science. Kildare Start With the Definition So, What Does “Biohacking” Actually Mean? There is no single universally accepted definition of biohacking. The term covers a surprisingly broad range of activities. At one end are familiar health behaviours: changing when you sleep, adjusting nutrition, exercising differently, tracking recovery or using wearable data to understand patterns. At the other end are experimental supplements, implants, unregulated compounds and technologies that may have very little evidence behind them. For ReWell, the useful part of biohacking sits firmly in the middle: understand what you are trying to improve, establish the fundamentals, use data where it adds useful information, selectively introduce appropriate interventions, and review what actually changes. A simpler definition:Biohacking is a deliberate, personalised approach to improving how you feel and function by making changes to lifestyle, environment or health routines — sometimes supported by data and technology — and then paying attention to the outcome. 01 Understand Start with what you are actually trying to change: energy, sleep, recovery, performance, healthy ageing or another clearly defined goal. 02 Measure Use relevant symptoms, habits, clinical information or carefully selected data rather than collecting numbers simply because you can. 03 Change Introduce an intervention with a clear reason for doing so rather than changing five things at once. 04 Review Ask whether it actually made a meaningful difference. If it did not, more technology is not automatically the answer. The ReWell View The Best Biohacks Are Often the Least Exciting Ones Biohacking can make health sound as though the body needs to be outsmarted. It doesn't. Many of the biggest influences on health, recovery and performance remain remarkably familiar. Before adding advanced technology, supplements or extensive testing, it makes sense to look at the foundations. Sleep Consistent, adequate sleep supports recovery, cognition, metabolic health and physical performance. Movement Regular aerobic activity, strength work and everyday movement remain fundamental to long-term health. Nutrition A sustainable eating pattern that meets individual nutritional needs usually matters more than chasing the latest supplement. Recovery Training load, rest, stress, hydration and recovery habits all affect how well the body adapts and performs. Build in the Right Order Think Foundation First. Technology Second. A useful biohacking approach is not about doing everything available. It is about knowing where each intervention sits in the bigger picture. Foundation Sleep, nutrition, movement, strength, recovery, stress management and appropriate medical care. Understand Your goals, health history, symptoms, current routine and the problem you are actually trying to solve. Measure Relevant clinical tests, functional information, wearables or other data where the result could genuinely influence what you do. Intervene Introduce appropriate lifestyle changes, nutritional strategies or technologies selectively. Review Look for meaningful change rather than assuming that because an intervention feels advanced, it must be effective. Tools & Technology Where Does Wellness Technology Fit? Technology can be useful, but each technology needs to be judged on its own evidence rather than being treated as effective simply because it falls under the “biohacking” label. HBOT Hyperbaric Oxygen Therapy uses increased atmospheric pressure together with oxygen exposure. It has established medical applications and is also being researched across areas including recovery, brain health and healthy ageing. The strength of evidence varies considerably by application. Red Light / PBM Photobiomodulation uses specific wavelengths of red or near-infrared light. Research spans a range of applications, but outcomes depend on wavelength, dose, device and the reason it is being used. PEMF Pulsed Electromagnetic Field technology uses electromagnetic fields and has been studied for a number of clinical and wellness applications. Evidence should be considered according to the particular device, protocol and intended outcome. Wearables Sleep, heart-rate, activity and recovery data can help identify patterns, but consumer wearable scores are estimates rather than diagnoses and should be interpreted in context. Functional Testing Testing can be useful when there is a clear question behind it and the result could alter a decision. More testing does not automatically mean better health information. Nutrition Personalising nutrition around goals, lifestyle and appropriate health information can be useful without turning food into a complicated optimisation exercise. More Data ≠ Better Data A test is useful when knowing the answer can help you make a better decision. Do You Need to Test Everything? No. One of the more questionable trends in modern wellness is the assumption that collecting more biomarkers, scans and biological data automatically leads to better health. Sometimes testing is extremely useful. Sometimes it creates a number that is difficult to interpret, has uncertain clinical relevance or prompts further investigations that were never needed. The better question is not: “What can we test?” It is: “What are we trying to understand, and would this result change what we do?” A useful test should have a purpose. At ReWell, functional testing is intended to add relevant information to an individual plan — not to create the impression that every aspect of health can or should be reduced to a dashboard of numbers. Explore Functional Testing Evidence vs Hype Not Everything Called a Biohack Is a Good Idea The biohacking world moves quickly. Social media can turn an early research finding into a “protocol” long before researchers know whether it meaningfully improves health in people. Supplements, peptides, extreme fasting protocols, large testing panels and new devices can all be marketed using scientific language without having strong evidence for the outcome being promised. Even when something has a plausible biological mechanism, that does not automatically mean it has been shown to improve health, performance or longevity. Four questions worth asking before trying a new biohack: What am I actually trying to improve? Is there credible human evidence for this outcome? What are the risks, costs and limitations? Biohacking is a broad term for deliberately changing aspects of your lifestyle, environment or health routine in an attempt to improve how you feel or function. It can include simple lifestyle changes, tracking data, nutrition and wellness technology. Is biohacking scientifically proven? Biohacking itself is not one treatment, so it cannot be labelled proven or unproven as a whole. Each intervention needs to be evaluated separately. Some practices have extensive evidence, while others remain experimental or poorly studied. Do I need expensive technology to biohack? No. Sleep, nutrition, physical activity, training, recovery and other everyday behaviours can be approached in exactly the same deliberate and measurable way without expensive equipment. Is HBOT a form of biohacking? HBOT is an established medical technology with recognised clinical applications and a growing research base in other areas. It is sometimes described as a biohacking or longevity technology when used in wellness settings, but the evidence varies considerably according to the reason it is being used. Can biohacking help with longevity? Some behaviours associated with the biohacking movement — such as regular exercise, good nutrition and adequate sleep — are relevant to long-term health. Evidence for many newer supplements, tests and technologies marketed specifically for longevity is still developing. No biohack can guarantee a longer life. Where should I start? Start with the outcome you care about and the fundamentals that influence it. If you are considering testing or wellness technology, ask whether it adds useful information or a realistic intervention rather than starting with the technology itself. The Bottom Line Good Biohacking Is Less About Hacking — And More About Understanding There is a useful idea underneath the biohacking label: pay attention to your health, understand what affects how you feel and function, make deliberate changes and learn from the outcome. But more data, more supplements and more technology do not automatically produce better health. Start with the fundamentals. Define the problem. Measure only where measurement adds value. Look at the quality of the evidence. Introduce interventions for a reason. Then review whether they genuinely made a difference. That is a far more useful approach than simply chasing the next “biohack”. Kildare Start With the Basics What Is PEMF Therapy? PEMF stands for Pulsed Electromagnetic Field Therapy. It uses a device to generate electromagnetic fields that switch on and off in controlled pulses. Unlike an electrical stimulation treatment, PEMF does not need to pass an electrical current directly through the skin. Instead, a changing magnetic field produces electrical effects within biological tissue. Different PEMF devices can operate at very different frequencies, intensities, waveforms and treatment schedules. That is important, because “PEMF therapy” is not one single standardised intervention. The simplest way to think about it:PEMF exposes tissue to carefully controlled, time-varying electromagnetic fields. Researchers are investigating how those fields interact with cell membranes, receptors and signalling pathways — and whether those effects translate into clinically meaningful improvements for particular conditions. The Science How Can an Electromagnetic Field Affect Tissue? The biology is more complex than the old explanation that PEMF simply “charges” cells or restores their natural electromagnetic field. Research suggests that electromagnetic stimulation may influence signalling at the cell membrane and affect pathways involved in cellular communication, extracellular matrix activity and inflammatory signalling. In bone and cartilage research, particular attention has been given to adenosine receptors and their downstream signalling effects. Cell-Membrane Signalling Electromagnetic fields may influence receptors and ion-dependent signalling processes at the cellular membrane. Adenosine Receptors Research in skeletal tissue has identified A2A and A3 adenosine receptors as one proposed pathway through which some PEMF protocols exert biological effects. Extracellular Matrix PEMF has been studied for effects on matrix-related processes in bone and cartilage, including pathways involved in tissue structure and repair. Inflammatory Signalling Experimental and clinical research suggests some PEMF protocols can influence cytokine and inflammatory signalling, although this varies substantially by application and device. Why Devices Differ PEMF Is Not One Single Treatment One of the biggest problems with broad claims about PEMF is that very different devices and protocols are often grouped together. A positive result from one PEMF study does not automatically mean a completely different home device or treatment programme will produce the same result. Frequency The number of electromagnetic pulses or cycles delivered over a given period can vary significantly between devices. Field Strength PEMF systems operate at different intensities. Higher intensity is not automatically more effective. Waveform Devices may generate different pulse shapes and electromagnetic patterns, affecting how the signal is delivered. Treatment Time Research protocols vary from short local treatments to longer sessions delivered repeatedly over several weeks. Where Is PEMF Being Studied? The Evidence Is Stronger in Some Areas Than Others PEMF has been investigated across many different areas of medicine, rehabilitation and wellness. That does not mean the evidence is equally strong for every use. Bone Healing Bone stimulation is one of PEMF's longest-standing clinical applications, particularly in difficult or delayed fracture healing and non-union. Osteoarthritis Knee osteoarthritis has been studied extensively, with some reviews reporting improvements in particular pain, stiffness or function measures — but results remain inconsistent and protocol-dependent. Musculoskeletal Pain Research includes low-back pain, shoulder conditions and other musculoskeletal presentations. Findings are encouraging in some studies but not uniform. Soft-Tissue Recovery Tendons, muscles and other soft tissues are areas of ongoing research, but the evidence is less established than it is for certain bone-related applications. Established Clinical Context PEMF & Bone Healing Bone healing is important because it provides a very different context from some of the broad wellness claims surrounding PEMF. Pulsed electromagnetic fields have been used clinically as bone-growth stimulation technology, including for difficult non-union fractures. Research into skeletal responses has identified plausible cellular pathways involving adenosine receptors, bone and cartilage matrix regulation and inflammatory signalling. Review · Bone Healing & Joint Preservation Cellular Mechanisms of Skeletal Response A major review of PEMF in skeletal tissue describes signalling through A2A and A3 adenosine receptors and dose-dependent effects on extracellular matrix components in bone and cartilage. These mechanisms provide a biological rationale for some established orthopaedic uses of PEMF. Important: evidence supporting PEMF in a particular bone-healing application cannot automatically be transferred to every pain, recovery or wellness claim. View the Research on PubMed → Musculoskeletal Research What About Pain? Pain is one of the most common reasons people encounter PEMF, but “PEMF reduces pain” is still too broad a statement. It is more useful to look at individual conditions. Systematic Review · 9 RCTs · 420 Participants Non-Specific Low-Back Pain A systematic review examined nine randomised controlled trials involving 420 people with non-specific low-back pain. The authors reported evidence supporting improvements in pain and physical function in some comparisons, but the included studies used heterogeneous treatment settings and protocols. The variation between studies means there is still no single universally accepted PEMF protocol for non-specific low-back pain. View the Review on PubMed → A Good Example of Nuance PEMF & Knee Osteoarthritis Osteoarthritis is a useful example of why we should avoid turning a mixed body of evidence into a blanket claim. 2026 Systematic Review & Meta-Analysis · 9 RCTs · 457 Patients Some Improvements — But Not Across Every Outcome A recent systematic review and meta-analysis included nine randomised trials involving 457 people with knee osteoarthritis. The researchers found no significant overall improvement in VAS pain or total WOMAC score at one month. However, some individual measures showed time-dependent improvements, including WOMAC pain at an earlier point and stiffness and daily activity at one month. Objective functional measures did not improve, and the authors noted that the overall risk of bias was high. Why this matters: the answer is not simply “PEMF works” or “PEMF doesn't work”. Outcomes appear to depend on what is being measured, when it is measured and which device parameters are being used. Established in Specific Uses PEMF has an established clinical history in certain orthopaedic applications, particularly bone stimulation. Promising in Other Areas Research across osteoarthritis, musculoskeletal pain and rehabilitation suggests potential benefit in selected populations and protocols. Not Proven for Everything Current evidence does not justify describing PEMF as a universal cellular-repair, healing, nervous-system or whole-body wellness treatment. Common PEMF Claims Mechanisms are useful — but they should not be turned into guaranteed health outcomes. What About ATP, Circulation & the Nervous System? PEMF marketing often says the therapy “recharges” cells, increases ATP, improves circulation and resets the nervous system. Experimental research does show that electromagnetic fields can influence cellular activity and signalling. But the leap from a laboratory mechanism to a guaranteed clinical outcome is much larger than those descriptions suggest. For example, evidence that a particular PEMF exposure affects a cellular signalling pathway does not prove that every PEMF session will increase a person's energy, accelerate healing or reduce stress. A useful rule Mechanism tells us how something might work. Clinical research tells us whether it actually produces a meaningful outcome in people. Good health information needs both. Safety Is PEMF Therapy Safe? PEMF is generally studied as a non-invasive intervention and recent musculoskeletal reviews report relatively favourable safety profiles. That does not mean every device is appropriate for every person. Device manufacturers may list specific contraindications or precautions, particularly around implanted electronic devices or other medical circumstances. Anyone with a pacemaker, implanted electrical or electronic device, significant medical condition, pregnancy or other reason for concern should obtain appropriate guidance before using PEMF. Device guidance matters. Different PEMF systems operate at different frequencies and field strengths. Safety information should therefore be considered for the actual equipment being used rather than treating every electromagnetic device as interchangeable. A More Useful Approach Where Could PEMF Fit Into a Wider Wellness or Recovery Plan? PEMF makes more sense when it is considered in relation to a defined goal rather than added simply because it is another piece of wellness technology. 01 Define Why You're Considering It Pain, rehabilitation, recovery and general wellness are not the same goal and do not have the same evidence behind them. 02 Look at the Evidence for That Goal Ask what has actually been studied in people with a similar problem — not simply whether there are “thousands of PEMF studies”. 03 Understand the Device & Protocol Frequency, field strength, waveform, duration and treatment schedule can all matter. 04 Keep the Fundamentals in Place PEMF should not replace appropriate diagnosis, medical care, physiotherapy, rehabilitation, movement, sleep or other foundational elements of recovery. The useful question is therefore not: “Does PEMF work?” It is: “Which PEMF protocol, for which condition, using which device — and what does the research show for that particular use?” That distinction is what turns PEMF from wellness hype into a technology that can be considered sensibly and in context.
